EXTRUDED DISC L4-5 WITH RIGHT FORAMINAL OCCLUSION.
Anamnesis
The patient a Saudi citizen came to the clinic

complaining of LBP for 6 months after lifting
heavy object with right
sciatica down with numbness right L5 and S1 territories.
The patient is hypertensive for 5 years under
treatment. MRI done 20-June-2002 showing
extruded disc L4-5 with right foraminal
occlusion.
On examination: The patient in agonizing pain,
limping with exaggerated scoliotic stance.
SLRS
20 degrees right side with pain. There is weak
dorsi and planterflexion right foot 3/5.
Right L5 foraminotomy with removal of the
extrusion lateral to the axilla. Right sided
intradiscal cleaning L4-5. The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak.
Routine closure of the wound.
Smooth postoperative recovery.
The power of the right foot improved.
He was sent to the ward.
Follow Up
The patient came to the clinic 14-August-2002
with clean wound and full power of the right foot
with hypalgesia right L5 territory.
Comments
There is still an estimated postoperative
recurrence around 7%.
