The patient was operated 10 years ago for PLD L4-5 for right sciatica. He was in relatively good condition when he started to complain of severe right sciatica 4 months ago for what he was operated 01-May-2005 elsewhere for huge recurrence of L4-5 right side. The patient continued to complain of the same complains and he was seen 16-June-2005 with agonizing pain and complete drop foot and weak planterflexion right foot. MRI done recently showing the same size of the recurrence still there and it is impossible to guess if it is the same fragment or de novo one. He was operated and foraminotomy of right L5 root was performed and the extrusion was removed in one piece.
