13-MAY-2014 SEHAM KHALEEL ABDEL-QADER 62 YEARS
RECENT EXTRUSION OF L4-5 WITH RIGHT FORAMINAL OCCLUSION.
Anamnesis
The patient came to the hospital 01-March-2014
complaining of LBP for 10 years with
exacerbation of LBP and right hip pain the last
2 weeks. The patient is a known diabetic and
hypertensive for 5 years in treatment. At that
time, the patient was not limping with no
scoliotic stance. SLRS was 75 degrees both sides
without pain. There was weak dorsiflexion right
foot 4/5. The patient was sent for
investigations the same day and MRI lumbar spine
showed bulge disci L3-4, L4-5 and L5-S1. The
patient was treated conservatively.
The patient then came 06-May-2014 telling that
the last 4 days got agonizing right sciatica
with numbness right foot.
On examination; the patient was limping with
exaggerated scoliotic stance. SLRS was 40
degrees with pain in the right side. There is
weak dorsi and planterflexion right foot 3/5.
MRI lumbar spine repeated the same day showing
extruded disc L4-5 with right foraminal
occlusion and downward migration.
The L4-5 level was identified with C-arm. Using
MICRODISCXS system with Carl Storz VITOM,
foraminotomy of the right L5 root was performed.
The extruded downward migrating disc was removed
in several pieces. Right sided intradiscal
cleaning L4-5 disc space.
Smooth postoperative recovery.
The power of the right foot became normal.
Comments
There is still an estimated postoperative
recurrence rate around 7%, because the disc space is still
not shallow.
