25-MARCH-2015 SHAFEEQ MUSTAFA MUHAMED 34 YEARS
EXTRUDED DISC L5-S1 WITH RIGHT DOWNWARD MIGRATION.
Anamnesis
The patient came to the clinic 22-March-2015
complaining of LBP for 6 years intermittent
course with agonizing right sciatica
down to the heel right foot for 5 days. MRI bad
quality done 21-March-2015 showing bulge disci
L4-5 and L5-S1.
On examination, the patient is limping with exaggerated scoliotic stance. SLRS
5 degrees with pain in the right and 40 degrees
with pain shooting to the right in the left
side. There is weak dorsiflexion right foot 3/5
and planterflexion same foot -4/5. The right AJ
is absent.
The patient was sent for investigations and MRI
done 23-March-2015 showing huge extruded disc L5-S1 with
right downward migration.
Foraminotomy right L5 and S1 roots. The extruded
disc L5-S1 was removed lateral to the axilla.
Right sided intradiscal cleaning L5-S1.
Routine closure of
the wound.
Smooth postoperative recovery. The power of right
foot became normal.
Comments
The patient still has an estimated
postoperative recurrence around 7%, because the disc space
still not shallow.
