EXTRUDED DISC L5-S1 WITH LEFT DOWNWARD MIGRATION.
Anamnesis
The patient came to the clinic 06-December-2004
complaining of LBP with left
sciatica for 3 days with left
sciatica down to all toes left
foot and positive cough sign. MRI lumbar spine performed
today showing extruded disc L5-S1
with left downward migration.
On examination: The patient is in agonizing
pain, limping with exaggerated scoliotic stance.
There is drop left foot with
planterflexion same foot. SLRS was 30 degrees
with pain left side.
Extended left S1 foraminotomy
with removal of the left downward migrating extrusion and left sided
intradiscal cleaning of L4-5. The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak. Routine
closure of the wound.
Smooth postoperative recovery.
The power of the left foot improved.
She was sent to the ward.
Follow Up
The patient came to the clinic 16-December-2004
with no sciatica and no senso-motor deficit. The
patient then came 20-April-2005 complaining of
four limb numbness , but neurologically very
with clinical picture for hypothyroidism. She
was sent to endocrine specialist.
Comments
There is still an estimated postoperative
disc recurrence around 7%, since the disc space is not
completely shallow.
