HUGE RECURRENCE OF L4-5 DISK WITH LEFT FORAMINAL OCCLUSION.
Anamnesis
The patient was operated buy me
11-May-2016 for extruded disk L4-5 with left
downward migration. The patient then came to me
11-July-2016 telling that she got LBP without
sciatica , but numbness with left knee pain.
On examination: the patient is limping due to
left knee pain. SLRS was 80 degrees left side
with tightness. There is no neurologic
deficit. The left knee is swollen and with
elevated temperature. The disk incision is
clean. ESR was 25 mm/h and CRP 10 mg/L.
The patient was sent for check MRI, which was
performed the same day and showing huge
recurrence of L4-5 with left foraminal
occlusion.
The old incision was
refreshed and neurolysis of the left L5 root was
performed. The extruded recurrence was removed
and further cleaning of the disk space of L4-5
was performed. The MultiGen was not used to see
if the effect of the previous application is
still present. Routine closure of the
wound.
Smooth postoperative recovery. The
patient was sent to the ward.
Comments
The patient still have an estimated
postoperative recurrence around 7%, because the disc space
is still not shallow.
The patient with recurrence after 2
months did not show an agonizing pain as during the first
visit before the 1st operation.
