The patient was operated by me 28-September-1999 for PLD L4-5 with right sciatica and drop right foot. The he came to the clinic 28-September-2003 complaining of LBP with right sciatica the last 4 months.

On examination: the patient is in agonizing pain, limping with exaggerated scoliotic stance. SLRS was 30 degrees right side with pain. There is almost drop right foot with weak planterflexion same foot with hypalgesia right L5 and S1 roots.

The patient was sent for investigations and MRI lumbar spine performed 02-October-2003 showing huge recurrence of L4-5 with right  foraminal occlusion.

The old incision refreshed. Neurolysis right L5 root. The first big piece was removed subaxillary. The other extruded disc material of L4-5 was removed from the right side with right sided intradiscal cleaning L3-4 was achieved. The patient was put in Reverse Trendelenburg position with Valsalva maneuver and hyperventilation. No CSF leak. Routine closure of the wound. Smooth postoperative recovery. The power of the left foot improved The patient was sent to the ward..

The patient came to the clinic 29-October-2003 with clean wound and weak dorsiflexion right foot 4/5.. SLRS was 75 degrees left side without pain.

The patient then came several times then he came 29-July-2017 telling that the last 2 months got LBP with bilateral sciatica .Using crutches for for 2months.He was sent for investigation and MRI lumbar spine showing severe stenosis L2-3 and L3-4.

There is still an estimated postoperative re-recurrence around 7% because the disc space not completely shallow.
