28-AUGUST-2012 SULAYMAN TAWFEEQ ATTA-ALLA 61 YEARS
EXTRUDED DISC L5-S1 WITH RIGHT DOWNWARD MIGRATION.
Anamnesis
The patient came to the clinic 22-August-2012
complaining of agonizing right sciatica with LBP
for 45 days with further exacerbation the last 3
days. The patient was operated for PLD L4-5 with
left sciatica 16 years ago.
On examination, the patient is limping with
exaggerated scoliotic stance. SLRS was 15
degrees in the right with pain. There is weak
planter and dorsiflexion right foot -4/5 with
hypalgesia right L5 and S1 territories. The AJ
is absent in the right.
MRI lumbar spine done 23-August-2012 showing
extruded disc L5-S1 with right downward
migration. There is small old bulge at L4-5 left
side of no significance.
Foraminotomy right S1 root.
The extruded disc was removed from under the
axilla. Right sided cleaning L5-S1 disc space.
Routine closure of the wound. Smooth
postoperative recovery and the power of the
right foot became normal.

Comments
Postoperative recurrence still above 7%, because
the disc space is still not shallow.
