12-NOVEMBER-2002 SULTAN MUHAMED AL-SARAJ 30 YEARS EXTRUDED DISC L4-5 WITH RIGHT FORAMINAL OCCLUUSION.
Anamnesis
The patient to the clinic
11-November-2002
complaining of LBP for 3 years with exacerbation
of right sciatica last 3 weeks. Positive cough sign.
MRI lumbar spine done the same day showing
huge disc L4-5 with right foraminal occlusion.
On examination: The patient in agonizing pain,
limping with exaggerated scoliotic stance.
SLRS
40 degrees
with pain in the right and 85 degrees in the
left shooting to the right. There is severe weak dorsi planterflexion
right foot 3/5, with hypalgesia right
L5 territory.
Right sided L5 foraminotomy with removal of the
extrusion lateral to the axilla from the
right. Right sided intradiscal cleaning L4-5.
Osteophytectomy done using drilling. The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak.
Routine closure of the wound.
Smooth postoperative recovery.
The power of the right foot improved.
He was sent to the ward.
Follow Up
The patient came to the clinic 24-November-2002
with clean wound and normal power and sensation
right foot with
SLRS 60 degrees without pain.
Comments
There is still an estimated postoperative
recurrence around 7%, because the disc space is still not
shallow.
