VERY HUGE EXTRUDED DISC L5-S1 WITH LEFT DOWNWARD MIGRATION.
Anamnesis
The patient came to the clinic 10-May-2015
complaining of left sciatica for 6 months with
exacerbation of the sciatica and LBP the last
month. She can sleep only in chair for 6 months.
MRI done 20-November-2014 showing huge extrusion
L5-S1 with left downward migration.
On examination; the patient is limping with
exaggerated scoliotic stance. Neck and upper
limbs were neurologically free. SLRS was 70 degrees with pain in the
left. There is weak dorsiflexion
left foot -3/5 and planterflexion same foot 4/5.
There is hypalgesia below the left knee. The
right AJ is absent.
The patient was sent for investigations and MRI
lumbar spine performed 12-May-2015 showing
further enlargement of the extrusion.
Using the C-arm, the L5-S1 level was identified.
Foraminotomy left S1 root was performed and
subaxillary removal of the huge extrusion was
achieved in one piece. Subaxillary left sided
intradiscal cleaning L5-S1 disc space. Routine
closure of the wound.
Smooth postoperative recovery. The power of left
foot became normal.
Comments
The patient still having an estimated
postoperative recurrence around 7%.
