EXTRUDED DISC L4-5 WITH RIGHT DOWNWARD MIGRATION.
Anamnesis
The patient came to the clinic
19-July-2012
complaining of LBP for 2 years and right
agonizing sciatica for
10 days.
MRI lumbar
spine done 18-July-2012 showing huge extruded disc
L4-5 with right downward migration.
On
examination, the patient is limping with
exaggerated scoliotic stance. SLRS was 20
degrees in right side with agonizing pain and 60
degrees in the left shooting to the right leg. There is
drop right foot and
weak planterflexion 3/5. There is numbness all
toes right foot with preserved sensation.
Right L5 foraminotomy with
extended far downward exposure under the axilla.
The extruded disc was removed from under the
axilla in several pieces. The L4-5 disc space
was cleaned from the right side.
Routine closure of the wound. Smooth
postoperative recovery. The power of the right
foot became normal.

Comments
The patient still have
postoperative estimated recurrence around 7%,
because the disc space height still not shallow.
