RECURRENT EXTRUSION L4-5 RIGHT SIDE.
Anamnesis
The patient was operated by me
16-December-2008 for huge extrude disc L4-5
for bilateral sciatica more the left and
bilateral intradiscal cleaning was done at that
time. The patient was doing well until she came
08-March-2013 complaining of right sciatica the
last week with LBP.
On examination: The patient is limping with
exaggerated scoliotic stance. SLRS was 5
degrees in the right with pain and 80 degrees in the left.
There is drop right foot 0/5 and severe weak
planterflexion 3/5 same foot with hypalgesia
right L5 and S1 roots. Before surgery there was
also weak dorsi and planterflexion of the left
foot.
The old wound refreshed.
Foraminotomy right L5 root with scarolysis. The
extruded downward migrating extrusion was
removed in several pieces. Right sided
intradiscal cleaning L4-5 disc space.
Routine closure of the wound.
Smooth postoperative recovery. The power of
the right foot became better.
Comments
There is still an estimated postoperative
recurrence below 7%, because the disc space is
still not shallow.
