10-SEPTEMBER-2014 TAHSIN KHALEEL ALI 35 YEARS VERY
HUGE EXTRUDED DISC L5-S1 WITH RIGHT UPWARD MIGRATION.
Anamnesis
The patient came to the clinic 08-September-2014
complaining of LBP with right
sciatica for 1 month with numbness of the IV and
Vth toes right foot. Cough sign positive. MRI
done 07-September-2014 showing very huge
extruded disc L5-S1 with right upward migration.
On examination; the patient is limping with exaggerated scoliotic
stance. SLRS was 30 degrees right side with pain.
Dorsi and planterflexion right foot -4/5. The
right AJ is absent.
Right S1 foraminotomy with partial flavotomy so
as to reach the upper limits from the right
side. The upward migrating huge piece was
removed in several big pieces. Right sided
intradiscal cleaning L5-S1 disc space. During
inspection of the surgical site before closure
another big piece was withdrawn from above and
removed. Routine
closure of the wound.
Smooth postoperative recovery. The power of the
right foot became normal.
Comments
The patient still has an estimated
postoperative recurrence around 7%.
