The recurrence rate in this case still
ranking around 14% because the disci spaces are not completely
collapsed.
Anamnesis
The patient a Yemeni citizen, came to the clinic
16-March-2003
complaining of LBP with bilateral sciatica more
the left for 3 years with exacerbation the last
20 days. MRI lumbar spine performed
15-March-2003 showing huge extruded disc L4-5
and small L5-S1 both to left side.
On examination: the patient in pain,
limping with exaggerated scoliotic stance. SLRS
was 35 degrees with pain left side. There is
almost drop left foot with weak planterflexion
same foot and hypalgesia left L5 and S1
territories.
Decompressive laminectomy L5,
upper edge of the sacrum and lower third of L4.
Foraminotomy both L5 and S1 both sides. The extruded disc
of L4-5 was
attacked from the right and right sided intradiscal
cleaning done. Left sided cleaning of
L4-5 was also done. The same was done
for L5-S1, but from the right side. The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak. Routine closure of
the wound
Smooth postoperative recovery.
The radicular pain decreased and the power of left
foot slightly improved. She was sent to the ward.
Follow Up
The patient came to the clinic 26-March-2003
with clean wound improved motor and sensory
functions. Still having weak dorsiflexion left
foot with hypalgesia left L5 root.
