YEARS EXTRUDED DISC L5-S1 WITH RIGHT FORAMINAL OCCLUSION.
Anamnesis
The patient came to the clinic 01-November-2004
complaining of LBP for 18 months with right
sciatica. 01-January-1998 suffered RTA with head
injury and hemothorax. Diabetic for 4 months.
MRI lumbar spine performed 12-May-2003 showing
extruded disc L5-S1 with right downward
migration.
On examination: The patient is limping with
exaggerated scoliotic stance. SLRS right side 45
degrees with pain. The right AJ is absent,
hypalgesia right L5 and S1 territories. Weak
dorsi and planterflexion right foot 4/5.
Right L5-S1 hemiflavotomy with foraminotomy of
the right S1 root with removal of the extrusion
and right sided intradiscal cleaning L5-S1 root.
The patient was put in Reverse Trendelenburg
position with Valsalva maneuver and
hyperventilation. No CSF leak. Routine closure
of the wound.
Smooth postoperative recovery.
The power of the right foot improved.
He was sent to the ward.
Follow Up
The patient came to the clinic 17-November-2004,
sciatica free and no motor, nor sensory deficit.
with clean wound.
The patient then came 05-March-2007 telling that
he got LBP for 3 months without sciatica. MRI
lumbar spine performed 04-March-2007 showed no
recurrence and treated conservatively.
Comments
The estimated postoperative recurrence at this
case is around 7%, because the disc space is not
shallow.
