The patient came to the clinic complaining of LBP for 4 years with the last 3 months having severe left sciatica with almost drop foot and weak planterflexion and hypalgesia right S1 territory. Left hemiflavotomy was performed and the extrusion was removed without violating the disc space. You can notice the discrepancy between the small size of the extrusion and the real huge one  after removal. Postoperative course was uneventful and the patient regained power.
