The patient came to the clinic 27-July-2003 complaining of both shoulders pain with weakness both upper limbs, then lower limbs. He was operated elsewhere 14-August-2002 with slight improvement followed with sudden deterioration after 3 months. MRI cervical spine before the surgery performed 28-July-2002 showed severe cervical canal stenosis C3-4, 4-5, 5-6 and C6-7. MRI performed 08-April-2003 before sudden deterioration showed still having the same stenosis.

On examination, the patient in wheelchair with spastic deep paraparesis and anesthesia with clonus and all pathologic reflexes with loss of urination and defecation control.

The patient was sent for investigation and MRI cervical spine done 12-August-2003 showing showing loose fixating device between C3 and C7 with upper screws broken.

Through the atypical scar, parallel to the right sternocleidomastoid muscle, the old incision refreshed and the loose construct was removed with parts of the broken screws.  Discectomy with osteophytectomy C3-4, 4-5,5-6 and C6-7 was achieved using drilling until the dura was relax at all levels. Using Scientx cervical plate bended to accept the curve of the area with 10 screws were applied to C3,4,5,6 and C7 level. Routine closure of the wound.

Smooth postoperative recovery. The patient was sent to the ward.

The patient came to the clinic 30-August-2003 with clean wound with various level of improvement and was advised to continue in medications and physiotherapy.

The cervical plates are susceptible to fractures as in this case.
