The patient came to the clinic 21-October-2002 complaining of neck pain for 2 months with both shoulder pain, more the left. Numb both upper limbs for 20 days and weak left upper limb with hypalgesia of the right side of the body below the nipple. (Brown-Sequard syndrome left side). Hypertensive for 20 years. MRI cervical spine done 15-October-2002 showing huge extruded disc C4-5 with stenosis C5-6.

On examination: There is mild kernicterus both eyes. Neck pain when rotating the head to left, up and down. Drop left wrist with severe weak grip left hand and triceps muscles both upper limbs. Rossolimo's sign positive left side. There is hypalgesia below the right nipple and weak all muscles left lower limb 4/5. All deep reflexes both lower limbs exaggerated with Babinski both sides and clonus more pronounced in the left foot. Dripping urine for 20 days.

Anterior approach with identification of C4-5 level by C-arm. Discectomy C4-5 with removal of the extrusion until the dura was seen relax at the bottom of the disc cavity. The same is done for C5-6. Using 2 level Scientx plate with 6 screws, fusion of C4-5-6 was achieved.  Routine closure of the wound.

Smooth postoperative recovery. The power of the left upper limb improved and he was sent to the ward.

The patient came to the clinic 05-November-2002 with clean wound, normal voiding, no drop left wrist and still slight weak left upper limb

The estimated postoperative recurrence is zero in cervical disci.

The myelopathic syndrome require several months to recover. In this case dramatic improvement took place because of relatively short duration of suffering.
