The patient came
to the
clinic 15-November-2006 complaining of LBP for 2 years with left
sciatica. MRI lumbar spine performed 11-September-2004 showed PLD
L4-5 with dehydration of L5-S1. Exacerbation of LBP the last 4
months with severe right sciatica. MRI done 04-November-2006 showed
huge extrusion of L4-5 with right mideolateral localization. On
examination: the patient walking with aid and had exaggerated
scoliotic stance with SLRS 0 degree in the right and
absent AJ, hypalgesia right L5 territory and weak dorsi (3/5) and
planter (4/5) flexion right foot. Right L4-5 hemiflavotomy with
right L5 root foraminotomy was performed and the huge extruded disc
L4-5 was removed in one piece lateral to the axilla. Minimal
cleaning of the disc space from the right side. Fat transfer to
cover the right L5 root. Routine closure. Prompt postoperative
recovery.
Comments:
1. Expectation for recurrence ranking in the middle, that is
because the disc space still relatively high and the defect in the
annulus fibrosis is relatively, not small. |
