MONTHS HYDROCEPHALUS DUE TO AQUADUCTAL STENOSIS.
Anamnesis
The patient came to the clinic 28-August-2004
with parents telling that he was full term with
normal delivery. They noticed enlarged head with
delayed milestones. The fontanel was 3X3 cm with
tense elevation. His H/C was 33 cm at birth. Now
the H/C is 50 cm. There is convulsion of the
right lower limb.
The patient was sent for investigation and MRI
of the brain done 11-September-2004 showing
aquaductal stenosis with severe progressive
hydrocephalus.
Insertion of PS Medical VPS
delta 1.5 performance small size was inserted to
the right anterior horn.
He was
sent to the ward.
FOLLOW UP
The patient came to the clinic 29-Septmber-2004
with lax fontanel and the shunt is functioning
and the wounds are clean.
The patient then came 14-December-2004 with new
MRI done 05-December-2004 showing diffuse brain
atrophy with wide subarachnoid spaces. The shunt
is functioning and the fontanel is closed. Had
one convulsion of the right lower limb. The
parents were advised to continue in
Phenobarbital.
The patient came several times and Hurler
syndrome was diagnosed and the last time seen
06-May2008 with distended abdomen with fever.
The shunt was functioning and he was sent to
pediatrician.
Comments
In case of progressive hydrocephalus, the sooner
the insertion of the shunt the better the
outcome.
