The patient after extensive exercises 2 months ago got left sciatica for what MRI done, showing bulging disc L4-5 and extruded disc L5-S1 left side with downward migration. The patient had weak dorsiflexion left foot with shooting sciatica. Left L5-S1 hemiflavotomy with left S1 root foraminotomy was performed and the extrusion was removed from under the axilla, after what the S1 root became from the laterally pushed position medial and relax. Cleaning of disc space was performed additionally lateral to the root.
