03-DECEMBER-2002 UMAR HAMZEH HAYMOOR 32 YEARS
LUMBAR CANAL STENOSIS L3-4, L4-5 WITH EXTRUDED DISC L4-5 MORE TO LEFT.
Anamnesis
The patient came to the clinic 03-November-2002
complaining of LBP with left sciatica for
45 days.
On examination: The patient not limping. SLRS
80 degrees both sides. There is weak
dorsiflexion both feet 4/5, with hypalgesia both
L5 territories.
The patient was sent for investigation: MRI
dorsal normal, lumbar showing severe stenosis
L3-4 and L4-5. There is mild extrusion L4-5 more
to the left.
Laminectomy of L4, lower third of L3 and upper
third of L5 with
foraminotomy both L4 and L5 roots. Left sided intradiscal cleaning L4-5.
Then right sided cleaning of L4-5 disc space. The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak.
Routine closure of the wound.
Smooth postoperative recovery.
The power of the feet improved.
He was sent to the ward.
Follow Up
The patient came to the clinic 14-December-2002
with clean wound and normal power and sensation with
SLRS 70 degrees without pain both sides.
The patient then came twice, then 15-April-2008
complaining of meralgia paresthetica left side
and was sent for investigations, but he
disappeared.
Comments
There is still an estimated postoperative
recurrence around 7%, because the disc space is still not
shallow.

Inomed Riechert-Mundinger System, with three point
fixation is the most accurate system in the market. The microdrive and
its sensor gives feed back about the localization.
