EXTRUDED DISC L4-5 WITH RIGHT FORAMINAL OCCLUSION.
Anamnesis
The patient came to the clinic 04-January-2015
complaining of LBP for 5 years with right sciatica. MRI
lumbar spine done 25-March-2014 showing
bulge disc L4-5.
On examination, the patient is in agonizing pain, limping with exaggerated scoliotic
stance. SLRS was 30 degrees with pain in the
right. There is weak dorsi and planterflexion
right foot -3/5. There is weak right quadriceps
4/5 with hypalgesia right L5 and S1 territories.
The patient was sent for investigations and MRI
lumbar spine done 04-January-2015 showing
extruded disc L4-5 with right foraminal
occlusion with bulge L5-S1. MRI dorsal spine
normal.
Using C-arm the level of L4-5 was identified and
foraminotomy right L5 root was performed. The
extruded disc was removed lateral to the axilla
and the downward migrating piece was removed and right sided intradiscal cleaning was done.
Routine closure of the wound.
Smooth postoperative recovery. The power of the
right foot became normal.
Comments
There is still an estimated postoperative
recurrence around 7%.
