POST-TRAUMATIC CENTRAL EXTRUDED DISC L4-5.
Comments
There is an estimated postoperative
recurrence around 7%, because the disc space is not
completely collapsed.
Anamnesis
The patient came to the clinic
05-May-2003 complaining of LBP for 8 months.
Felt down 6 weeks ago after what got
exacerbation of LBP with left sciatica with
fracture S1. The patient had trauma to the neck
1986 and was operated by me.
On examination: the patient is in agonizing
pain, limping with exaggerated scoliotic stance.
SLRS was 30 degrees with pain right side and 40
degrees with pain left side. There is hypalgesia
left S1 root with weak dorsi and planterflexion
left foot 3/5.
The patient was sent for investigations: MRI
done 08-May-2003 showing extruded disc L4-5 with
central downward migration.
Bilateral L4-5 flavotomy with
foraminotomy both L5 roots. Bilateral removal of the
extrusion and cleaning of the disc space. The
patient was put in Reverse Trendelenburg position
with Valsalva maneuver and hyperventilation. No CSF
leak. Routine closure of
the wound
Smooth postoperative recovery.
He was sent to the ward.
Follow Up
The patient came to the clinic 17-June-2003 with
clean wound, but pain left loin and SLRS 50
degrees right side with pain and 40 degrees with
pain left side with no motor, nor sensory
deficit.
The patient then came 20-July-2003 with SLRS 70
degrees with pain right side and 80 degrees
without pain left side. He has stiff back.
The patient then came several times and the last
one 22-January-2012, telling that he was in good
condition, but the last 2 months got LBP and
right sciatica. MRI lumbar spine performed
19-January-2012 showing no recurrence at the
site of surgery, but there is huge extrusion
L3-4 with right downward migration. He was given
admission, but he disappeared.
