Comments
There is still an estimated postoperative
recurrence, because the disc space still not shallow.
Anamnesis
The patient came to the clinic 10-April-2002
with agonizing LBP with bilateral sciatica, more
to the left. MRI lumbar spine done 09-April-2002
showing huge extruded disc L4-5 with bulge
L5-S1.
On examination: The patient in agonizing pain,
limping with exaggerated scoliotic stance. SLRS was
55 degrees with pain
left side. There is no sensory deficit
but weak dorsi and planterflexion left foot
-4/5.
Bilateral foraminotomy L5
roots with removal of the extrusion from the
left side, then bilateral cleaning of L4-5 disc
space. The patient was put in Reverse
Trendelenburg position with Valsalva maneuver
and hyperventilation. No CSF leak. Routine closure of the
wound.
Smooth postoperative recovery.
The power of the left foot improved.
He was sent to the ward.
Follow Up
The patient came to the clinic 18-April-2002
with clean wound with no sensory or motor
deficits.
