The patient has 4 months history of LBP with right sciatica with drop right foot with decreased sensation over the L5 territory. MRI showed huge PLD L4-5 . Bilateral flavotomy L4-5 was done with foraminotomy of right L5 root was done after what it was possible to remove the huge extrusion in almost one piece. Further cleaning of the disc space was performed. The trend now is to minimally clean the intradiscal content to avoid intradiscal weakening of the structure and to avoid possible discitis.
