26-MARCH-2011 UMAR ABDEL-LATIF
AL-BASHITY 52 YEARS RESIDUAL AFTER ANTERIOR DISCECTOMY WITH
SUBSEQUENT STENOSIS C4-5. 5-6 AND C6-7.
Anamnesis
The patient
was operated by me
06-September-2010
for PCD C4-5, 5-6 for quadriparesis after what
he got considerable improvement. The patient
then came 21-March-2011 claiming that he got
deterioration the last 2 weeks with numbness of
both lower limbs with difficult walking.
On
examination, the patient has hypalgesia below
the left D4 with weak right lower limb 4/5 of
the proximal and distal muscles. The patient
sent for investigations.
MRI
cervical
spine performed 22-March-2011 showing severe
cervical canal stenosis at C4-5, 5-6 and C6-7.
with malacia of the spinal cord. The major
compression is from behind.
Decompressive laminectomy of C4-5-6 and C7.
There was no epidural fat at these levels.
Routine
closure of the wound. Smooth postoperative
recovery with improvement of the power of right
leg.

Comments
The patient was operated in the first time for
PCDs, but the cages inserted to give more height
to the disc spaces to decrease the stenotic
elements of the spinal canal failed after
sinking of the PEEK inside the cavity.
In this case the second operation is mandatory
to eliminate the stenosis.
