The patient came to the clinic 15-May-2005 with numbness and left
sciatica for one month. The patient was operated for PLD L4-5 1980.
The patient is a known hypertensive with insulin-dependant diabetes
mellitus. On examination, there was hypalgesia of the left S1 root
with severe weak dorsi and planterflexion all toes left foot with
scoliotic stance. MRI performed and showed recurrence at the
same level with huge extrusion left side with downward migration.
The patient was operated: left L5 foraminotomy with release of
the root from the adhesions. The extruded disc was stuck to the root
and it was necessary to remove parts of the extrusion from the side
of the axilla and part from under the axilla.
