The patient came to the clinic 19-July-2003 complaining of numbness both hands with neck pain for several months with difficult walking. The condition is progressing. The patient is a known diabetic for 17 years and hypertensive for 18 years.

On examination: the patient has pain in the neck when looking to right and up. There is hypalgesia right upper limb and both limbs below the knees. There is weak grip and extension both hands and both triceps muscles. Weak dorsi and planterflexion both feet more weak left.

The patient then came 08-November-2003 with MRI cervical spine showing severe stenosis due to extruded disc C3-4, 4-5 and C5-6.

Using C-arm, discectomy of C3-4, C4-5 and C5-6 was done until the dura was seen at the entire posterior border of the bodies. Using 3 level cervical plate fusion of C3-4-5-6 was done.

Routine closure of the wound. Smooth postoperative recovery. The power of four limbs became better.

The patient came to the clinic 24-December-2003 with clean wound and the sensory deficit regressed to the tip of the index finger right hand and the dorsum of the right foot.

The patient came 13-November-2005 10 days after suffering CVA. MRI of the brain done 05-November-2005 showing infarction right parietal lobe. The patient now complaining of LBP with weak dorsi and planterflexion both feet. She was sent for cardio consultation and thorough investigations. 

The patient has severe stenosis due to extruded disc C3-4, 4-5 and C5-6. Decompression and fixation is the best solution for this situation.
