LUMBAR CANAL STENOSIS L2-3, 3-4 4-5 WITH SPONDYLOLISTHESIS L3-4 AND L4-5.
Anamnesis
The patient came to the clinic 01-November-2015
complaining of LBP for 4 years without sciatica.
Exacerbation of LBP with bilateral sciatica the
last month more the right.
On examination: the patient is limping with
exaggerated scoliotic stance. SLRS was 80 degrees without pain
both sides. There is weak dorsiflexion right
foot 3/5 and left -4/5 and planterflexion right
foot 4/5.
The patient was sent for investigations and MRI
done 01-November-2015 showing lumbar canal
stenosis L2-3, 3-4, 4-5 and spondylolisthesis
L3-4 and L4-5.
Transpedicular fixation
L3.4.5 using XIA 3 system with 6 monoaxial
screws 6.5x45 mm and bended rods 6 mm with MAC
42 mm. All stages were done with C-arm.
Decompressive laminectomy L3,4 and upper fourth
of L5. Foraminotomy L3,4,5 roots both side. Using MultiGen, a pulsed mode
RF with 42 Celsius, 240 sec, 2 Hz and 20 mesec
duration the L3,4,5 roots both sides was achieved.
The harvested bone graft was milted and applied
lateral to the rods. Routine closure of the
wound.
Smooth postoperative recovery.
The power of both feet became normal. The patient was
sent to the ward.

Comments
The patient has multiple problems in the
lumbar spine which need surgical correction.
This is the 18th case using the PRF mode
with MultiGen.
