YEARS SEVERE CERVICAL CANAL STENOSIS C3-4, C5-6, C6-7 WITH MYELOPATHIC
SYNDROME.
Anamnesis
The patient came to the clinic
02-August-2002 complaining of neck pain
with four limbs weakness for 8 months. The last
2 months exacerbation
of neck and left upper limb weakness. MRI
cervical spine performed 17-July-2002 showing
huge extruded disc C3-4, C5-6 and C6-7 with
severe malacia of the spinal cord, more behind
C3-4.
On examination: The patient has neck pain when
rotating the head to all directions. There is weak
left deltoid, biceps, grip,
extension left hand 3/5 and the right triceps
muscle 3/5 and weak all muscles lower limbs up
to 4/5. There is hypalgesia of the median
distribution left hand. There is fasciculation
both deltoid muscles.
Trans-nasal intubation.
Through anterior approach, using the C-arm the level of C3-4 identified
and discectomy with osteophytectomy was
performed until the dura was seen at the bottom
of the created disc space. The same was done for
C5-6 and C6-7. Using Scientx 3 level plate with
6 screws fusion of C5-6-7 was achieved. Routine
closure of the wound.
Smooth postoperative recovery.
The power of four limbs slightly improved.
He was sent to the ward.
Follow Up
The patient came to the clinic 18-August-2002
with clean wound and further improvement of the power of
all
limbs and no radicular pain.
Comments
The cervical disci never recur.
When 2 adjacent cervical disci removed fusion
is a must.
Skyra MRI with all clinical applications in the run since 28-Novemeber-2013.

its sensor gives feed back about the localization.
