The patient came to the clinic 02-September-2003 complaining of LBP and right sciatica for 3 years. Operated for right knee 5 months ago. Hypertensive for 2 years, in treatment. MRI lumbar done 26-September-2002 showing LCS L4-5.

On examination: the patient walking with aid, limping with wide based legs gait. She has tetraparesis four limbs with weak grip, extension and triceps right upper limb 3/5. Hoffman positive both sides. SLRS was 30 degrees both sides with pain. Brisk KJ both sides with Babinski positive  with weak dorsiflexion both feet more the right. All the muscles of the lower limbs are weak.

The patient was sent to perform MRI of the brain with contrast and MRI of the cervical spine, which was done 14-September-2003 to show a very huge extruded disc C5-6 with malacia of the spinal cord.

Discectomy of C5-6 was done until the dura was seen along all the posterior aspects. All stages of surgery were done under C-arm control.

Routine closure of the wound. Smooth postoperative recovery. The power of the four limbs improved.

The patient came to the clinic 29-September-2003 with clean wound and the power of the four limbs improved, except the drop left foot.

The patient then came 17-December-2003 complaining of inability to walk properly due to cramps of the right lower limb with almost drop right foot.

The patient then came several times with ups and downs then came 10-September-2005 with MRI lumbar spine showing stenosis D9-10. L3-4, 4-5 and L5-S1.

The patient has extrusion of C5-6 causing the tetraparesis. She mostly suffered for more than 3 years, for what recovery will not be dramatic. but at least to stop deterioration.
