22-MAY-2013 WISAM AHMAD DAAS 46 YEARS
RECURRENT EXTRUSION L4-5 WITH RIGHT FORAMINAL OCCLUSION.
Anamnesis
The patient came
to the clinic 13-May-2013 complaining of LBP for
three months with right sciatica after three
days with numbness of all toes right foot. The
patient was operated by me 15-September-1999 for
PLD L4-5 right side with subsequent improvement.
On examination: The patient is limping with
exaggerated scoliotic stance. SLRS was 90
degrees both sides with tightness. There is drop
right foot with weak planterflexion 0/5.
Right L5 foraminotomy. The L5
root is scareous and severely deformed. The
extruded disc was removed from under the axilla.
The root which was pushed laterally, was further
dissected and right sided cleaning of L4-5 disc
space was achieved.
Routine closure of the wound.
Smooth postoperative recovery.
The power of right foot became normal.
Comments
The patient has recurrence after 14 years. He
still has an estimated postoperative
re-recurrence around 7%.
