YEARS HUGE EXTRUDED DISC L3-4 LEFT SIDE WITH SEGMENTAL STENOSIS L4-5.
Anamnesis
The patient came to the clinic 31-March-2003 complaining of LBP
since 1984. The last month got exacerbation of
LBP with
left
sciatica. MRI done
24-March-2003 showing huge extruded disc L3-4
more to the left with old extruded disc L4-5. The patient is hypertensive for
one week in
treatment.
On examination: the patient is in agonizing
pain, limping with exaggerated scoliotic stance.
SLRS was 30 degrees left side with pain. There
is weak dorsiflexion both feet with urgency and
frequency of bladder function for 1 month.
Decompressive laminectomy
L3-4 and L4-5 with foraminotomy L4 and 5 roots
both sides .The very huge piece was removed from
lateral to the left L4 root. Left sided L3-4
discectomy lateral to the
axilla and left sided intradiscal cleaning of
L3-4 space was achieved. Inspection of L4-5 disc
was not significant, for what it was not
violated. The patient was put in Reverse Trendelenburg
position with Valsalva maneuver and
hyperventilation. No CSF leak. Routine closure
of the wound.
Smooth postoperative recovery. The power of the
left foot normalized.
FOLLOW UP
The patient
came to the clinic 20-October-2003 with clean wound
and the power of the left foot normal.
Comments
There is still an
estimated postoperative recurrence around 7%
because the disc space is not completely shallow.
