YEARS EXTRUDED DISC L4-5 WITH LEFT FORAMINAL OCCLUSION.
Anamnesis
The patient came to the clinic 21-April-2014
complaining of left sciatica for 10 years with
exacerbation the last 5 months with numbness big
toe left foot. MRI lumbar spine bad quality,
done 09-April-2014 showing extruded disc L4-5
with left foraminal occlusion.
On examination; the patient is limping with
exaggerated scoliotic stance. SLRS was 90
degrees with pain in the left. There is weak
dorsiflexion left foot -3/5 with hypalgesia left
L4,5and S1 roots. MRI of the lumbar
spine repeated the same day confirming the
previous data with left far-lateral extrusion.
Using the C-arm, the L4-5 level identified.
Using MICRODISCXS system and VITOM Carl Storz
attachments, foraminotomy left L5
root was performed and the huge extruded disc
was removed lateral to the axilla. Left sided
cleaning L4-5 disc space. The root hanging free.
Routine closure of the wound.
Smooth postoperative recovery. The power of the
left foot became normal.
Medicon MICRODISCXS set.
Comments
The patient has still have an estimated
postoperative recurrence around 7%, because the
disc space is still not shallow.
The MICRODISCXS system is just a retractor
system, it must be used in conjunction with
VITOM to obtain better video recording.
