YEARS HUGE EXTRUDED DISC L4-5 MORE TO THE RIGHT.
Anamnesis
The patient came to the clinic 17-June-2002
complaining of LBP for 1 week and right
sciatica.
On examination: The patient is limping with exaggerated scoliotic stance.
He cannot set down and cannot sleep due to pain. SLRS was
15 degrees with
pain in right side and 50 degrees with less pain
in the left. There is weak dorsi and
planterflexion right foot
3/5 with hypalgesia right L5 and S1 territories.
There is also weak left foot dorsi and
planterflexion 4/5.
The patient was sent for investigation: MRI
lumbar spine performed 19-June-2002 showing very
large extrusion L4-5 with right maximum and left
downward migration.
Bilateral L5 foraminotomy with removal of the
extrusion lateral to the left axilla and left sided
intradiscal cleaning L4-5. After that right
intradiscal cleaning. The patient was put in Reverse
Trendelenburg position with Valsalva maneuver
and hyperventilation. No CSF leak. Routine
closure of the wound.
Smooth postoperative recovery.
The power of the right foot normalized.
He was sent to the ward.
Follow Up
The patient came to the clinic 06-July-2002
with clean wound and sciatica free and normal
power right foot with occasional numbness right
L5 territory. SLRS was 70 degrees with mild pain
in the right.
Comments
There is an estimated postoperative
recurrence around 7%, because the disc space is still not
shallow.
