YEARS EARLY DISCITIS L4-5 LEFT SIDE.
Anamnesis
The patient was operated by me 24-April-2014 and
was discharged 2 days with good condition. The
last day got agonizing left sciatica, for what
MRI repeated showing fullness of the area with
possible soft disc fragments extrusion. The
patient is from Yemen and time is critical in
this situation. The patient was admitted and a
trail for conservative treatment failed for what
exploration of the wound was decided.
On examination; the patient is limping with
exaggerated scoliotic stance. SLRS was 60
degrees with pain in the left. There is no
weakness. CRP was 85 mg/dL and ESR 20
mm/h.
The wound opened. A gush of pus came out from
under the facia and it was coming fro the disc
space. It was sent for CXS. Cleaning of the disc
space from the discitis material. The edematous
soft tissues were cleaning, to eliminate any
source of compression. Meticulous irrigation
with saline, Gentamicin and a powder of Maxipime
1 gm was filled to the operative field and
routine closure of the wound. Nylon stitches
were used for the cutaneous layer.
The patient is a diabetic, but such rapid
escalation of the events with pus collection
within several hours, despite the fact the the
patient was covered empirically with 2
antibiotics, is rare but possible.
The patient must have low immunity, that
triggered such event.
