Comments
The estimated postoperative recurrence is
around 7%, because the disc space is still not completely
shallow.
Anamnesis
The patient came to the clinic
07-May-2003 complaining of LBP for 4 years
with left sciatica for
1 year with exacerbation the last month with
numbness of the entire left foot. MRI lumbar spine performed 24-April-2003
showing huge extruded disc L5-S1 more to the
left.
On examination: the patient is limping with
exaggerated scoliotic stance with SLRS 20
degrees left side with pain with hypalgesia left
L5, S1 roots with weak dorsi and planterflexion
left foot 4/5.
Bilateral L5-S1 flavotomy with
foraminotomy both S1 roots. The extrusion was
attacked first from the left side lateral to the
left S1 root and a huge pieces of extrusion were
removed. Then right sided cleaning of L5-S1 disc
space with alternated left to right cleaning until
both roots became lax. The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak. Routine closure of
the wound
Smooth postoperative recovery. He was sent to the ward.
Follow Up
The patient came to the clinic 21-May-2003 with
clean wound with no neurological deficit. SLRS
was 50 degrees without pain in the left.
The patient then came 13-July-2008 complaining
of LBP without sciatica for 1 year. MRI lumbar
spine performed 30-June-2008 showing extruded
disc L3-4. He was neurologically free and sent
for investigation. MRI lumbar spine done
16-July-2008 showing shrinkage of the extrusion
and he was treated conservatively.
