HUGE SUBACUTE SUBDURAL HEMATOMA RIGHT CEREBRAL CONVEXITY.
Anamnesis
The patient came to the Emergency of Shmaisani
hospital with clinical history of falling down 1
month ago with cut wound the left brow. He is a
known diabetic with arterial hypertension in
medications. He was in acceptable condition,
when he yesterday progressed ataxic gait and
felt down twice.
CT-scan done immediately, showing huge
extradural hematoma right cerebral convexity,
over the right frontal and parietal lobes.
On examination: the patient is unable to walk
with pronounced left sided hemiparesis. The
patient cannot remember the events one month ago
and cannot tell when the paresis got place.
In side lying position, one burr hole done over
the right frontal region and the other at the
most posterior aspect of the right parietal
region. The dura was opened and the subdural
hematoma was evacuated and the external drain
was inserted to the parietal region. Washing of
the hematoma was done from the frontal burr hole
with N/S. Routine closure of the wounds.
Smooth postoperative
recovery. The power of left side became normal
and the patient sent to the ICU.
Comments
The patient has huge subdural hematoma. Surgical
evacuation is the only solution.
The patient removed the drain 4 hours after
surgery.
