10-DECEMBER-2012 YOUSEF IBRAHEEM ABU-SAYF 28 YEARS EXTRUDED DISC
L5-S1 WITH FAR RIGHT DOWNWARD MIGRATION.
Anamnesis
The patient came to the clinic 08-December-2012
complaining of LBP for 8 months with right
sciatica for 4 months down to the lateral
malleolus. Using crutches for one week. MRI bad
quality done 11-September-2012 showing small
extrusions L4-5 and L5-S1.
On examination: the patient
is limping with exaggerated scoliotic stance
with SLRS 20 degrees with pain in the right. The
patient is in agonizing sciatica and there is
weak dorsiflexion right foot -4/5.
New MRI of the lumbar spine
done 08-December-2012 showing huge extruded disc
L5-S1 with far right downward migration and
bulge L4-5.
The patient had very huge fat
in the back, for what special instrumentation
was used. Foraminotomy of the right S1 root was
achieved and the extruded disc was removed
lateral to the axilla. Right sided cleaning of
L5-S1 disc space.
Routine closure of the
wounds. Smooth postoperative recovery.
The power of the right foot became normal.

Comments
The patient still have an
estimated postoperative recurrence below 7%,
because the disc space height is shallower than
the average.
