36 YEARS HUGE EXTRUDED DISC L3-4 WITH RIGHT DOWNWARD MIGRATION
Anamnesis
The patient came to the clinic 22-April-2014
complaining of left sciatica for 4 years with
exacerbation the last 3 months with inability to
stand and walk in the morning. MRI lumbar spine
done 29-February-2014, bad quality, showing huge
extruded disc L3-4.
On examination; the patient is limping with
no apparent scoliotic stance. SLRS was 80
degrees right side with pain shooting to the left
and 30 degrees in the left with more pain. There is weak
dorsiflexion left foot -3/5.
MRI of the lumbar spine repeated the same day
showing huge extruded disc L3-4 central
more to the right
with right downward migration.
Using the C-arm, the L3-4 level identified.
Laminectomy of the lower 2/3 of L3 and upper
third of L4 with bilateral foraminotomy both L4
roots was done. The huge extruded disc with
downward migrating pieces was removed from the
right. Bilateral intradiscal cleaning of L3-4
disc space with removal of the compressing
osteophytes.
Smooth postoperative recovery. The power of the
both feet became normal.
Comments
The patient has still have an estimated
postoperative recurrence around 7%, because the
disc space is still not shallow.
The use of endoscopic techniques in this
circumstance is not adequate, because the task
was for wide decompression.
