YEARS SPONDYLOLISTHESIS L3-4 WITH STENOSIS AT L2-3 AND L3-4.
Anamnesis
The patient came to the clinic 17-August-2015
complaining of LBP for 3 years with left
sciatica with exacerbation last three months
with pareasthesia both lower limbs. She cannot
walk more than 150 meters.
On examination: the patient was limping with scoliotic stance.
SLRS 40 degrees with pain both sides. There is
weak dorsiflexion both feet 4/5 and abduction
and adduction of the knees both sides 4/5.
The patient was sent for investigations and MRI lumbar spine
performed 17-August-2015 showing spondylolisthesis L3-4 with
severe stenosis at this level and L2-3. Dynamic studies
confirmed this.
Skeletonization of L2,3 and
L4. Transpedicular fixation L3 and L4 was
achieved using XIA 3 Stryker screws 6.5x45 mm
with bended rods 5.5x60 mm Decompressive
laminectomy of L3 and 4. Foraminotomy both L4 roots.
Using MultiGen with PRF mode applied to both L4
roots with the following parameters: 240 sec, 42
Celsius, 2 Hz 20 millisecond. MAC Cross connector
42 mm applied and the harvested bone milted and
applied lateral to the rods. Routine closure of the
wound.
Smooth postoperative recovery. The power of
both legs became normal. The patient was
sent to the ward.

Comments
The patient has complete stenosis with
spondylolisthesis. The injurious process of this condition
will escalate with time. The sooner the surgical
intervention, the better the results.
