The patient came to the clinic 24-July-2014 complaining of left sciatica for 5 weeks and LBP for 3 weeks. The patient using crutches for 1 year and walking with help. The patient underwent 15 and 10 years ago disc surgery for the spine and she is diabetic for 3 years.

On examination; the patient is limping with exaggerated scoliotic stance. SLRS was 85 degrees with pain in the left. There is old drop feet with weak planterflexion both feet 4/5 with hypalgesia left S1 root.

The patient was sent for investigations: MRI lumbar spine showing severe degenerative spine with stenosis at D10-11, 11-12 and D12-L1  with malacia of the spinal cord with mild spondylolisthesis L4-5 without overmobility. It was explained to the relatives that surgery will not improve her condition. The patient urged to at least relief her left sciatica because a lot of medications are not easing her condition and she cannot sleep due to pain.

Using C-arm, with the patient awake, the junction of the inferior border of the left L3 transverse process and lateral mass was identified and Xylocain with Doprofos  were injected to reach the left L3 root. Similar approach was done for the left L4, L5, S1 roots same side. 80 mg Doprofos with 40 ml 1% Xylocain were used.

The patient tolerated the procedure well and FBS before the procedure was 230 mg/dL and 125 mg/dL after the procedure.

The patient will not improve from surgery. The agonizing sciatica could be eased by this procedure.
