07-AUGUST-2003 ZAYD HABES DAMEN 38 YEARS EXTRUDED DISC L4-5 WITH RIGHT DOWNWARD
MIGRATION.
Anamnesis
The patient came to the clinic
05-August-2003 complaining of LBP for 4 days with
right sciatica to the right L5 territory. MRI
lumbar spine done today showing extruded disc
L4-5 with right downward migration.
On examination, the patient is limping with
exaggerated scoliotic stance. SLRS was 80
degrees right side with pain. There
is drop right foot and hypalgesia right L5
territory.
Right L4-5 hemiflavotomy with
foraminotomy right L5 root. The extruded disc was
removed lateral to the right axilla in huge
big piece and right
sided intradiscal cleaning L4-5 was achieved.
The patient was put in Reverse Trendelenburg
position with Valsalva maneuver and
hyperventilation. No CSF leak. Routine closure of the wound.
Smooth postoperative recovery.
The patient showed improvement of the power of
his right foot.
He was sent to the ward.
FOLLOW UP
The patient
came to the clinic 19-August-2003 with clean wound.
SLRS was 45 degrees with pain right side, but the
weakness of the right foot is 4/5 and no sensory
deficit. The patient was advised to continue in
medical treatment and advised to perform
physiotherapy to strengthen the power of the right
foot.
Comments
There is still an estimated postoperative
recurrence less than 7%, because the disc space
is shallow, but not completely collapsed.
