HUGE EXTRUDED DISC L4-5.
Anamnesis
The patient's brother came to the clinic 08-December-2004
telling that the patient is
complaining of severe LBP with bilateral
sciatica for 6 days. MRI lumbar spine performed
04-December-2004 showing very huge extruded disc
L4-5.
On examination before surgery: The patient is in agonizing
pain, limping with exaggerated scoliotic stance.
There is weak dorsiflexion both feet 2/5,
planterflexion same foot 3/5. SLRS was 10 degrees with pain
both
sides.
Decompressive laminectomy L4-5
with bilateral L5 foraminotomy
with removal of the huge extrusion from right
subaxillary approach and bilateral
intradiscal cleaning of L4-5. Due to severe
compression, the tiny dura was stitched by nylon 6
zero to avoid postoperative CSF leak. The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak. Routine
closure of the wound.
Smooth postoperative recovery.
The power of the feet improved.
He was sent to the ward.
Follow Up
The patient came to the clinic 20-December-2004
with weak dorsi and planterflexion right foot
4/5. The patient then came 24-July-2012
complaining of left sciatica with full
power of the right foot and weak dorsiflexion
left foot 4/5. He was sent for investigations,
but he never showed up again.
Comments
There is still an estimated postoperative
disc recurrence around 7%, since the disc space is not
completely shallow.

fixation is the most accurate system in the market. The microdrive and
its sensor gives feed back about the localization.
