The patient came to the clinic 12-April-2016 complaining of weak left leg for 1 year  with weak left upper limb the last 2 weeks. MRI cervical spine performed 04-April-2016 showing huge extruded disc C4-5, 5-6 and C6-7 with spinal cord compression and malacia of the spinal cord. MRI of the brain was normal and lumbar spine showed old wedge fracture L1 with bulge disci L3-4, 4-5 and L5-S1 of no clinical significance.

On examination: the patient is limping as dragging the left lower limb. There is neck pain when turning the head to the left and looking up. The left deltoid was almost 0 and the left biceps -4/5. Hand flexion -4/5, extension 3/5 and the left triceps -3/5. The left shoulder was frozen. DTR showed exacerbation on the left side.  SLRS was 30 degrees due to weakness with exaggerated deep reflexes in the left and Babinski positive in the left. There is weak dorsiflexion right foot 4/5. dorsiflexion left foot -4/5 and planterflexion same foot 4/5.

Diskectomy with osteophytectomy C4-5, 5-6 and C6-7 until the dura was seen al over. Using Stryker Reflex III level plate 48 mm with 4 variable 16 mm screws to C4 and C7 and 4 fixed screws to C5 and C6 with Solis 2 cages 14x12x5 mm to C5-6, 6-7 and one cage 14x12x6 mm to C4-5 with bone graft, fusion of C4-5-6-7 was achieved. All stages of surgery were guided with C-arm. Routine closure of the wound.

Smooth postoperative recovery. The power of the lower limb normalized and considerable improvement of the power of the left upper limb.

The patient needs diskectomy of all levels, since huge diski were noted at them.
