Anamnesis
The patient was admitted to Al-Shmaisani
hospital with history of LBP for one month and
agonizing left sciatica for one week. MRI
performed 19-November-2007 showing huge extruded
disc L4-5 left side.
On examination: the patient is irritable, cannot
find suitable position, with drop left foot and
weak dorsiflexion with hypalgesia left L5 and S1
roots territories.
Left L4-5 hemiflavotomy with left L5
foraminotomy was performed and the extruded disc
was removed in one piece lateral to the axilla.
Meticulous cleaning of the disc space from the
left. Routine closure of the wound.
Smooth postoperative recovery with normalization
of the power of left foot.
Comments
The expected recurrence rate is around 7%
because the disc height is still not noticeable and the
annulus fibrosis defect is not small.
