YEARS TRANSPEDICULAR FIXATION AFTER REMOVAL OF THE OLD CAMILLE-ROY FRACTURED CONSTRUCT.
Anamnesis
The patient a Libyan citizen came to the clinic
14-August-2002 complaining of LBP the last 7
months with left sciatica down to the left
L5 territory. She was operated 1985 for
spondylolisthesis L4-5 and fusion using Roy
Camille plate and screws were used with
bilateral bone fusion. The device was broken
1991. She is insulin dependent diabetic for 6
years and hypertensive for 13 years.
On examination: The patient in agonizing pain,
liming with exaggerated scoliotic stance, SLRS
was 60 degrees with pain left side. There is
weak dorsi and planterflexion both feet -4/5.
The patient was sent for investigations and
spondylolisthesis L3-4 was identified.
The old construct was removed and transpedicular
fixation of L3-4 was performed. Decompressive
laminectomy L3-4 was done with foraminotomy both
L4 roots. The patient was put in
Reverse Trendelenburg position with Valsalva
maneuver and hyperventilation. No CSF leak.
Routine closure of the wound. Routine
closure of the wound.
Smooth postoperative recovery.
The power of both feet normalized.
She was sent to the ward.
Follow Up
The patient came to the clinic 04-September-2002
with clean wound and full power of the feet.
Comments
The Roy Camille plates are no longer used
for fixation of the spine. In recent time the best option is
to undergo transpedicular fixation to mobile segment, t.e,
the L3-4.
