The patient got left sciatica without LBP for 4 months, with exacerbation of the sciatica the last month. On examination he had almost drop left foot with weak planterflexion of the same foot. SLRS was 10 degrees in the left. MRI performed, showing huge extrusion of L5-S1 disc with downward migration.

The patient underwent left L5-S1 hemiflavotomy with foraminotomy of left S1 root. The extrusion was removed from under the axilla in one piece. Minimal cleaning of the disc space considering that, the disc material was looking like more or less a healthy tissue.
