PROGRESSING HYDROCEPHALUS DUE TO TROUBLESHOOTING VPS.
Anamnesis
The patient was operated by me
12-January-2015 for progressive
hydrocephalus by inserting medium pressure VPS.
The patient continued to complain after then
with lesser degree. MRI of the brain and CT-scan
was done 23-April-2015 showing short ventricular
tip of the shunt, that when it evacuate the CSF
the tip becoming non functioning, for what it
was decided to replace this part with longer
one.
The old incision refreshed and extended
posterior to expose the bur hole site and the
proximal connection of the reservoir. The
reservoir is placed far from the bur hole, the
reason why the ventricular end was short. The
ventricular part of the shunt was removed and
another one with different direction more
vertical to the plane and projecting to the
right pupil and the right inner ear planes. It
was pushed 7 cm and the CSF came clear. Another
3 cm were advanced without stylette. The
ventricular part was connected to the reservoir
aided with 4 zero nylon stitch. Routine closure
of the wound.
Smooth postoperative recovery. The patient was
sent to the floor.
Comments
The patient with far positioned reservoir
caused the ventricular tip to be short. All the holes of the
ventricular tip must be in the ventricular cavities to work
adequately.
